Resumen:
 

La vulvodinia es un problema bastante más frecuente que lo que se puede percibir en las consultas de médicos de familia o ginecólogos.

Se trata de un síndrome de dolor vulvar inexplicable, con frecuencia acompañado de discapacidades físicas, limitación de las actividades diarias, la disfunción sexual y la angustia psicológica. El dolor en la vulva de la paciente por lo general tiene un comienzo agudo y, en la mayoría de los casos, se convierte en un problema crónico que dura meses o años. El dolor es a menudo descrito como ardor o escozor, o una sensación de aspereza o irritación.

La vulvodinia (Terminología ISSVD 2003) puede estar relacionada con un trastorno específico (infeccioso, inflamatorio, neoplásico o neurológico) o tratarse de una vulvodinia esencial. La vulvodinia esencial puede ser generalizada o localizada y aparecer de forma espontánea o provocada.

La evaluación debe incluir una historia detallada y un examen físico, así como cultivos de bacterias y hongos, y biopsia de cualquier área sospechosa.

El tratamiento ha de adecuarse a la identificación del tipo concreto de vulvodinia. Dependiendo del diagnóstico específico, el tratamiento puede incluir el fluconazol, antibioterapia,  citrato de calcio, antidepresivos tricíclicos, corticoides tópicos, terapia física con el biofeedback, la cirugía o láserterapia. Dado que la vulvodinia es a menudo una condición crónica, regular un seguimiento médico y la conjunción de un grupo de apoyo son útiles para la mayoría de las pacientes.
